We Support Families

with clear guidance through every step of the transition

When a loved one needs assisted living, families are often navigating unfamiliar decisions under emotional pressure. Divine Transitions for Seniors provides calm, professional guidance through three essential parts of the transition — helping families move forward with clarity and dignity.

Placement Guidance Support

Helping families find the right care community

Choosing assisted living isn't just about availability — it's about finding the right fit.

We guide families through the process of identifying assisted living or memory care communities that align with their loved one's care needs, personality, lifestyle, and budget. Our role is to help families understand their options, ask the right questions, and move forward with confidence — without pressure or rush.

What this includes:

  • Understanding care needs and preferences

  • Identifying appropriate assisted living or memory care options

  • Supporting families through tours and decision-making

  • Helping families feel confident in their choice

Home Transition Support

Helping families find the right care community

For many families, the sale of a senior's home becomes the funding source for assisted living. This can add stress to an already emotional process.

We help families plan and coordinate the home transition so timing, logistics, and expectations feel manageable. While we do not sell real estate ourselves, we assist families in navigating this step thoughtfully and connect them with trusted professionals when needed.

What this includes:

  • Planning the home transition timeline

  • Coordinating next steps to prepare the home for sale

  • Supporting families through the emotional aspects of letting go

  • Helping ensure the home transition aligns with care needs

Personal Belongings Support

Sorting through a lifetime of belongings can feel overwhelming — especially during a major life change.

We support families in creating a thoughtful plan for personal belongings, helping them decide what to keep, relocate, share, donate, or manage through estate services. Our approach is always respectful, calm, and guided by the family's wishes.

Handling life's possessions with care and respect

What this includes:

  • Creating a plan for personal belongings

  • Supporting downsizing and relocation decisions

  • Coordinating resources when additional help is needed

  • Reducing emotional and logistical overwhelm

Our services are

always free to families.

In some cases, Divine Transitions for Seniors may receive referral compensation from service providers. This does not affect the family's cost or their freedom to choose.

Reviews

  • “She worked on our timeline — and was there through it all.” Madena supported our family through nearly a decade of transition — from helping my parents move into a safer home, to guiding us through my mom’s memory care journey after my dad passed. What stood out most was that she worked on our timeline, not hers. She was a calm, steady presence during some of our most difficult moments, and because of her, my mom is now safe and beautifully cared for.

  • “She was the calm in the middle of complete chaos.” When my mom had a stroke, I was 150 miles away dealing with my own health issues and an upcoming surgery. Madena happened to be there — she stepped in immediately, took my mom to the hospital, and didn’t leave. What followed was overwhelming — coordinating rehab across state lines, legal paperwork, facility tours, and medical transport. Madena handled details I couldn’t manage from a distance and made sure my mom was always cared for with dignity. She brought order to chaos and was a steady, trusted presence every step of the way.

About

Hi, I'm Madena.

At my core, I've always been someone who advocates for others — especially during times when life feels overwhelming and decisions need to be made quickly.

My work is deeply personal. I've supported four of my own family members through senior transitions, walking through the emotional and logistical challenges that come with navigating care, the home, and everything in between. Each experience was different, bringing its own unique challenges and decisions — something that showed me how personal and nuanced every transition truly is.

I've seen firsthand how quickly things can become overwhelming and how difficult it can be for families to manage it all on their own. Families get stuck — not because they don't care, but because there are so many moving pieces and not enough support.

That's why I created Divine Transitions for Seniors.

Today, I help families navigate the home, belongings, and logistics side of transitions — bringing structure, clarity, and calm so they can move forward without feeling overwhelmed. When circumstances change quickly and the home must be sold to fund care, I coordinate the expedited home sale and help families create a thoughtful plan for personal belongings. Families are never charged for my services.

Beyond the Business

Earlier this year a friend asked me to check on a senior couple here on the Long Beach Peninsula. When I visited and assessed the situation — a husband doing his best to care for his wife after a stroke, calling paramedics simply because he couldn't safely help her up — I realized this probably wasn't a one-off. There had to be others out there quietly struggling the same way.

So I posted on a local community Facebook group and asked a simple question — is this unusual, and if not, what can we do about it?

The response was overwhelming. 271 reactions and 104 comments in 12 hours.

Out of that conversation, I founded the Divine Connection Network.

DCN is a volunteer organization connecting isolated seniors on the Long Beach Peninsula with community support through two programs:

Neighborhood Senior Watch — neighbors keeping a caring eye on senior neighbors, so no one slips through the cracks quietly.

Adopt-a-Senior — one-on-one volunteer connections that bring consistent presence, support, and dignity to seniors who may have limited family nearby.

The Long Beach Peninsula is a uniquely beautiful and uniquely vulnerable community. Nearly half of our population is over the age of 65 — yet senior care resources here are extraordinarily limited. Seniors who need assisted living or memory care often have to leave the Peninsula entirely to find it. Families are spread across states. And the isolation that comes with that gap is real and daily.

DCN exists because Divine Transitions for Seniors exists — and Divine Transitions for Seniors exists because of what I saw when I walked through that front door.

This work doesn't stop when the business day does.
